Promoting Cultural Competency in the Care of Women and Communities Affected by Female Genital Cutting

There are large numbers of refugees and immigrants in the US from countries that practice Female Genital Cutting (FGC). As a result, US health and social service providers must rapidly adapt to meet the growing and unique needs of this population. Of those women and girls at risk, 31,000 reside in the Midwest Region V of the US Department of Health and Human Services (Illinois, Indiana, Michigan, Minnesota, Ohio, Wisconsin). The Midwest Network on FGC (MNFGC) is an organization committed to working with health providers and community members to raise awareness about the practice, prevent it from occurring, and to improve care for those who have been cut. This poster and DVD presentation will highlight the small but growing body of evidence that suggests FGC-affected communities are vulnerable to health access and outcome disparities related to their FGC status. The Primary Health Care model is examined as a theoretical and a practical approach to caring for FGC-affected women. Specific strategies, such as the culturally-specific videos, provide a concrete example of how to provide specialized care to this population. Highlighted work includes Dr. Crista Johnson's recent study in Columbus OH on Somali immigrant experiences in US health care, Dr. Priscilla Flynn's development and implementation of Somali prenatal education videos to improve communication between patients and providers, and Dr. Nicole Warren work with the Heartland Health Center’s African Women’s Clinic in Chicago where a Primary Health Care model provides the theoretical framework for providing culturally and clinically appropriate care for FGC-affected women.

      Biographical Sketch:
      Nicole Warren, PhD, MPH, CNM - Nicole was lived and worked in Mali, West Africa where FGC is nearly universal. As a certified nurse-midwife, Nicole has also cared for FGC-affected women in the US in both antepartum and intrapartum settings. She has presented widely on how to provide culturally and clinically appropriate care to this population. She is currently working on a study to understand Ethiopian women's reproductive health experiences and helping to organize Heartland Health’s African Women’s Clinic schedule to open in March of 2008.

      Biographical Sketch:
      Priscilla Flynn, MPH, CHES, DrPH is currently the Coordinator for the Office of Women’s Health in Rochester, Minnesota. She received her master of public health in community health education from the University of Wisconsin-LaCrosse, and her doctor of public health in health care policy and administration from the University of North Carolina-Chapel Hill. Her experience in public health includes working to improve the health of underserved women through improved access to care and increased coordination of existing health and human service organizations in SE Minnesota. Dr. Flynn speaks nationally on Somali refugee women’s issues including prenatal education, birth outcomes as affected by acculturation and the provision of culturally competent care.

      Biographical Sketch:
      Crista Johnson, MD, Msc is a Clinical Lecturer and Robert Wood Johnson Clinical Scholar in the Department of Obstetrics & Gynecology at the University of Michigan Health System. She received her undergraduate degree from The Johns Hopkins University, medical degree from the Weill Medical College of Cornell University, and completed her residency in Obstetrics & Gynecology at the George Washington University Medical Center. She subsequently completed a fellowship in Female Sexual Medicine at the University of California, Los Angeles which led her to explore the cultural implications of Female Genital Cutting on psychosexual sequela. She has presented nationally on the challenges faced by health care providers in the care of women with FGe. Dr. Johnson's current research focuses on investigating strategies to improve health outcomes for immigrant women who have undergone FGC by improving health care access and utilization, reproductive health education, counseling, and health care provider cultural competency.

      Biographical Sketch:
      Celine Woznica has over thirty years’ experience in the formation of health promoters, both in Latin America and in Chicago, Illinois. Currently,she directs a refugee health promoters program on the north side of Chicago serving refugees who have been in the U.S. for over thirty years as well as very recently arrived refugees. She also heads a coalition of five refugee resettlement agencies, three clinics, and other community- based organizations that serve refugees. As associate director of Heartland Health Outreach’s Refugee Health Programs, Dr. Woznica oversees health promoters, doulas, a refugee nutrition program, and multi-lingual health education development. Another interest is the inclusion of non-English speaking immigrants and refugees in emergency preparedness planning.
